Social Prescribing

Working with healthcare professionals and local services, we offer inclusive music activities that help people feel more confident, connected, and supported. Our projects provide a safe, welcoming space for anyone experiencing loneliness, anxiety, or health challenges to improve their wellbeing through music.

What is social prescribing?

Social prescribing recognises that practical, emotional, and social support can make a real difference to people’s lives. It connects individuals to local activities, groups, and services that help them feel supported, listened to, and connected to their community, improving both wellbeing and quality of life.

 

By working alongside traditional healthcare and clinical pathways, social prescribing offers a more holistic approach to care, helping to prevent issues from escalating while also reducing pressure on overstretched NHS services and enabling people to take a more active role in their own health and recovery.

1. Referral – A healthcare professional or another agency refers a person to a link worker.

2. Personalised Plan – The link worker spends time understanding the individual’s needs and co-creates a tailored care and support plan.

3. Connection – The link worker links the person with suitable community groups and services.

4. Support – The individual is supported to take control of their own health and wellbeing through these non-medical interventions.

SoundLincs' Social Prescription Service

SoundLincs provides a creative health alternative for people experiencing social isolation, anxiety, depression, or other challenges to their mental health. Through structured, music-based activities, participants can rebuild confidence, develop coping strategies, and form meaningful social connections.

 

Referrals can be made via social prescribers, health professionals, voluntary sector partners, or through self-referral. Each participant receives a SoundLincs consultation to assess their personal needs and musical interests before being matched with one of our trained music facilitators. Our interventions include one-to-one support, small group sessions, and community outreach events, all co-designed with participants and partners to meet local needs.

Activities take place in accessible community venues such as wellbeing hubs and community centres, with home-based sessions also available where transport or mobility may be a barrier.
